How Sleepy Are You?

How likely are you to doze off or fall asleep in the following situations?

You should rate your chances of dozing off, not just feeling tired.

  • No chance of dozing = 0
  • Slight chance of dozing = 1
  • Moderate chance of dozing = 2
  • High chance of dozing = 3

Write down the number corresponding to your choice in the margin, or make notes of which ones. Total your score below.

Chance of Dozing

Sitting and reading

Watching TV

Sitting inactive in a public place (e.g., a theater or a meeting)

As a passenger in a car for an hour without a break

Lying down to rest in the afternoon when circumstances permit

Sitting and talking to someone

Sitting quietly after a lunch without alcohol

In a car, while stopped for a few minutes in traffic

Total Score

Interpretation – This is an indicator of your risk for having Obstructive Sleep Apnea (OSA).

0-6: It is unlikely that you are abnormally sleepy.

7-8: You have an average amount of daytime sleepiness.

9-15: You may be excessively sleepy depending on the situation. You may want to consider seeking medical attention for possible OSA.

16-24: You are excessively sleepy and should consider seeking medical attention for probable OSA.

Other Indicators of Obstructive Sleep Apnea

Do you snore? Yes No

Do you have high blood pressure? Yes No

Do you grind your teeth at night? Yes No

Do you awaken from sleep gasping or choking? Yes No

Has anyone noticed you stop breathing at night? Yes No

Do you have headaches when you awaken? Yes No

Is your neck size 17+” (male) or 15.5+” (female)? Yes No

Are you more than 15 pounds overweight? Yes No

Answering Yes to any 3 of these questions indicates an increased probability that you have OSA.

Untreated OSA increases your risk of having a stroke, a heart attack, congestive heart failure,

hypertension, arteriosclerosis, diabetes, impotence, GERD and other chronic diseases
